Preventable patient harm continues to occur daily in hospitals and health care systems.  Achieving high reliability is essential to reducing and preventing patient harm.  Aviation and nuclear power are examples of other industries that have achieved safety levels despite their high-risk environments and healthcare must apply these same principles and do the same.  We must turn our focus to system improvement vs. reactive event analysis and ensure the execution of every process is reliable.
